USAHR collects personally identifiable information when you register for a “registration” account, when you visit pages on registry.USAHR.org, and when you enter promotions conducted on registry.USAHR.org. USAHR may also collect personally identifiable information when you visit or register with a site maintained by an affiliate of USAHR. USAHR may also receive personally identifiable information from other sources. When you register for an account on registry.USAHR.org, we may ask for your name, e-mail address, birth date, telephone number, gender, zip code, occupation, industry, and personal interests. Once you register on registry.USAHR.org and sign in to our services, you are not anonymous to us. USAHR also automatically receives and records information on our server logs from your browser including your IP address, USAHR cookie information, and the page you requested.

In some cases, USAHR uses contact forms for visitors who choose to use these forms to provide information and request information. We collect your willingly submitted contact information (such as your email address, name, address and phone number). Contact information from these response forms is used to get in touch with the visitor when necessary in order to resolve customer service issues or questions.
